It seems a very wise idea to recommend Vitamin D supplementation both before and after any surgical procedure to improve recovery time and repair. Getting your Vitamin D level checked as part of the pre-op lab work-up also seems a wise decision. <br />

This study does not prove that having a good Vitamin D level promotes healing, but it sure does seem to suggest it. Supplementing Vitamin D in yourself or a loved one about to undergo any type of surgery, or after an injury, sounds like a very cheap and safe way to protect and heal tissue, with virtually no down-side.<br />

You are what you eat; computer programmers have a saying, "Garbage in, garbage out." That saying also applies, I think, to the foods you put inside your body, and the bodies of your family. The boring, day-to-day choices you make about your diet add up to the body and the health you will enjoy.</div><br />
Some tidbits to remember, potatoes and ketchup are <u>not</u> vegetables, peanut butter is <u>not</u> a meat, peanuts are <u>not</u> nuts, and for my Southern friends, deep-fried farm-raised catfish is <u>not</u> a fish...<br />

A <b><u><span class="Apple-style-span" style="color: blue;"><a href="http://jama.ama-assn.org/content/304/18/2042.short">recent study in JAMA</a></span></u></b> shows that allowing your child to become obese could curse them with a lifetime of adult obesity.<br />
<br />
This study followed about 80 obese children and found out that (surprise) they grew up to be obese adults. <br />
Of course this study doesn't mean these children will have no choice in the matter later on should they be obese in childhood, it simply means the likelihood of being obese later in life is increased. As adults we are all responsible for our own health, but as parents you are in charge of your child's diet and lifestyle.<br />
<br />
So, as parents, it is your duty to clear your mind of all the ads for sugary cereal and "fun" places for kids to eat, and rememeber that your job is to raise a healthy, capable adult human who can then in turn raise your grand-children properly. Here are a few thoughts that are important in this process:<br />
<br />
<ul><li><b>Buy Real Food</b> - As the grown-up in the house you buy the groceries, so stop buying the junk! If <iframe align="right" frameborder="0" marginheight="0" marginwidth="0" scrolling="no" src="http://rcm.amazon.com/e/cm?t=medinplawor-20&o=1&p=8&l=bpl&asins=B003JPJ21Q&fc1=000000&IS2=1&lt1=_blank&m=amazon&lc1=0000FF&bc1=000000&bg1=FFFFFF&f=ifr" style="align: right; height: 245px; padding-right: 10px; padding-top: 5px; width: 131px;"></iframe>it's not in the cabinets or in the frig, they will not be able to eat it any longer. You shouldn't be cruel and make every needed change over night, but starting today, gradual, continuous changes need to be made. When a child grows up eating raw fruits, veggies, berries and lean cuts of healthy chicken, turkey and fish, they think this is how you are suppossed to do it, and that's how they do it as a grown up. If a daily trip (or two) to Mc-Wendy-King's is part of their childhood, they won't be able to picture an adult-hood without it. If the cabinets and fridge are filled with good choices, it is not cruel of you to allow your kids to skip a meal or two in protest, or for them to actually feel some real hunger-pangs. When they get truly hungry, they will gobble up all the tasty, healthy food you have made available. A good rule of thumb: If you see it advertised on TV, it's probably not real food.</li>
</ul><div class="separator" style="clear: both; text-align: center;"></div><ul><li><b>Set a Good Example</b> - As the parent, you are setting the example for the next generation, your children. I see folks in the office all the time who think they have bad "genetics", who in reality just learned to eat like their parents, who made bad choices regularly. These patterns and habits have become so hard-wired into their brain they can't even imagine a healthy junk-free lifestyle as a possible reality. </li>
<li><b>Train the Elephant While He is Young</b> - The fable goes, when a baby elephant begins life with the circus, he is tied to a stake in the ground with a regular-sized piece of rope. Try as he might, he cannot break this rope and thus "learns" that it is too strong to break. When he is grown, he still "know"s he can not break this rope and doesn't even try. If a grown elephant not raised in this manner where tied with this same rope, he would snap it effortlessly and walk away. The diet and lifestyle examples you set during the toddler and childhood years are hard-wired into your child's brain and will become part of their adult life. Remember, they are watching you all the time, even when you think they are not.</li>
<li><b>Food is fuel, not entertainment</b> - There is nothing wrong with an occasional piece <iframe align="right" frameborder="0" marginheight="0" marginwidth="0" scrolling="no" src="http://rcm.amazon.com/e/cm?t=medinplawor-20&o=1&p=8&l=bpl&asins=1933653728&fc1=000000&IS2=1&lt1=_blank&m=amazon&lc1=0000FF&bc1=000000&bg1=FFFFFF&f=ifr" style="align: right; height: 245px; padding-right: 10px; padding-top: 5px; width: 131px;"></iframe>of Birthday Cake or a rare visit to Mc-Wendy-King's, but these should be rare treats, not daily (or even weekly) expectations. Television and Magazine ads attempt to indoctrinate your children to want these things all the time for one reason, to make money. The huge corporations are not interested in raising healthy children, they are interested in raising their quarterly profits. Minimize the number of ads your children see when possible and explain to them why theses huge companies make such commercials. Kids catch on quickly to the idea of commercials being a trick. Abby Grace (under 10 years old as of this writing) was intrigued by the idea that the Clown on TV was an actor and trying to "trick" her into wanting more pseudo-chicken nuggets. Recently, after watching a commercial portraying a lucky little green fellow adding yet another color of marsh-mellow to an already sugar-coma cereal she said, "They'r just tryin' to make more money, aren't they Daddy?" </li>
<li><b>Live the Lifestyle</b> - If you turn off the TV and actually appear to enjoy going outside and playing with your kids, they will think this behavior is normal. There's nothing wrong with watching the occasional show with your kids, but TV should not define your life or your lifestyle. In order to think that being a healthy, active adult is normal, they need to actually see you living that lifestyle. Preaching it but not living it will not work.</li>
<li><b>Give Them a Multi-Vitamin</b> - Get your children used to the idea of taking a supplement when they are young. I see little evidence of the American diet changing much in the next couple of decades, so they will have to take supplements in order to get the micro-nutrients and minerals they need while living in this multi-national-corporation controlled world. The American diet is virtually devoid of Vitamin D, so that should be one of their first supplements.</li>
<li><b><img alt="" border="0" height="1" src="http://www.assoc-amazon.com/e/ir?t=medinplawor-20&l=bil&camp=213689&creative=392969&o=1&a=B003BVIBMO" style="border: none !important; margin: 0px !important; padding: 0px !important;" width="1" />Demand they get Plenty of Sleep</b> - Just as you are responsible for what foods are found in your kitchen, you are also in charge of the light-switch, the TV and all the electronic devices that rob your child of the growth-promoting, obesity-fighting benefits of a full night of deep sleep. Kids need to be in bed by at least 9 pm to have full advantage of the melatonin-cortisol-growth hormone-insulin benefits of a quiet, dark, peaceful sleep environment. The fact is, a full night's sleep in a dark, quiet bed is as necessary for long-term health as anything else. And, like most truly good things, it is free.</li>

You would think that a concept such as nut-and-seed-induced diverticulitis would have quickly fallen by the wayside in the face of the overwhelming research findings against it, and with absolutely no research supporting the false idea. There are many such medical myths, widely believed by both doctors and patients, sadly, the doctors often profit from such myths while the patients sometimes suffer.<br />

Once stopped up, the little pouch would become inflamed and lead to an attack of diverticulitis. For several years I thoughtlessly repeated this medical myth to my own patients. This is a perfect example of how a Medical Myth is created and perpetuated, without any real supporting facts. This idea/mental image seemed to make so much sense, it must be true. Well guess what? Not only is this Medical Myth untrue, it appears that the exact opposite is true, according to a very large study, eating seeds, nuts and popcorn actually decreases one's risk of having diverticulitis.<br />

But still, I have patients returning from their visits with a gastroenterologist reporting they have been told to avoid food containing nuts and seeds. I would encourage anyone about to keep an appointment with a gastroenterologist to print out a copy of this <b><u><span class="Apple-style-span" style="color: blue;"><a href="http://jama.ama-assn.org/content/300/8/907.full.pdf+html">study</a></span></u></b> and take with them in order to <i>educate </i>the doctor just in case they are given such "advice". So, what did this very large, very long study find actually caused diverticulitis? That's a very good question.<br />

Now we can't do much about getting older, but we can definitely take charge of and improve our weight to height ratio (BMI). We can add diverticulitis to the long list of bad things caused by smoking. We can certainly increase our physical activity, and cut down on the total amount of over-the-counter pain relievers we take.<br />

</tbody></table>Cookware coated with Teflon and similar products, at least initially, make cooking easier. But, I have yet to buy a coated skillet and have the coating last for very long. Where does the missing coating actually go? Did we eat it one fleck and molecule at a time? What is it made of? Just how bad is it? What about if one of the kids over-heats the skillet after it has been scratched??? So, I just went out and bought us a nice big Lodge cast-iron skillet and commenced to "season" it. After a month or so of attempts by me (everyone else abandoned the heavy thing much sooner) I gave up trying to make a pretty egg in it and when back to the Teflon-devil coated skillet. Recently, I was very happy to learn a thing or two about the art and science of cast-iron cooking. <br />

Grandmother's old cast iron skillet (the best kind) contains no harmful chemicals at all, and is actually better than a new skillet. You could actually grind up the skillet and eat it with no ill effects, except maybe to your teeth. The blacker the skillet is, the better it will cook. The shinier the surface is, the more non-stick it will be. <br />
<br />
But here is the medical factoid of the day; cooking in cast iron actually adds quite a bit of natural elemental iron to your diet. How much? It depends on what you are cooking. At the bottom of this article is a short list I found very interesting, it comes from a 1986 article in The Journal of The American Diabetic Association. It is quite possible that the iron skillet which traveled across America with each pioneer family saved the life of many a wife and mother who otherwise would have died from complications of iron-deficiency anemia.<br />

Everything you cook in cast iron has more iron in it after the cooking, some things have substantially increased levels. Tomato based recipes really pull the iron into the food being prepared, I'm assuming its the acidity that does this. The obvious question, "If the skillet is giving up all this healthy iron with each meal cooked in it, and it has been used for decades, why has the skillet not disappeared???" I'm not sure why that doesn't happen but I'm glad it doesn't. But, what about my less than impressive experience with my new skillet? Some of you have tried to cook in cast iron and found the results to be less than expected. The skillets you can buy new today are very heavy and the cooking surface is very rough and neither of these traits endears the skillet to it's owner. I have found out that, as usual, people had more sense in past years, and that skillets used to be lighter and have much smoother cooking surfaces.<br />

Do NOT go out and buy a heavy, new rough-surfaced cast iron skillet or you will be disappointed. If you are not lucky enough to have inherited a good skillet, get on Ebay.com and bid on the blackest skillet you see made by Griswold. These Made-in-the-USA skillets are a little lighter, and the cook-surface is much smoother. The blacker skillets have been treated right through the years and will be virtually stick-proof. A new skillet must be "seasoned" and if the surface is too rough, this can be quite the frustrating episode in your kitchen-career.<br />

People who find that taking Iron Supplements gives them nausea or constipation will be happy to find they can get the same amount of iron from their skillet without those symptoms. Iron from a skillet is elemental and is thus immediately absorbed, almost always without any side effects. A recent <u><a href="http://www.dailymail.co.uk/health/article-1354683/Junk-food-diet-hits-childs-IQ-reveals-major-UK-study.html"><span class="Apple-style-span" style="color: blue;">article appearing in the London newspaper<b> The Daily Mail</b></span></a></u> reported the results of a very large study performed in the United Kingdom claiming to show that eating Junk Food prevented children from achieving the IQ they could have otherwise. As you may know, I am interested in how improving nutrition helps us be better people, so I was intrigued, and read this one with great interest.<br />

The data for this study was harvested from the <a href="http://www.bristol.ac.uk/alspac/"><span class="Apple-style-span" style="color: blue;">Avon Longitudinal Study of Parents and Children</span></a> (<i>Avon=the river/county in England, not the cosmetic</i>). This huge study looked at many aspects of children, their health, habits and diet. It took years to compile and sort through all the data they collected during this study. Researchers are still going through it to discover new information.<br />

Now, as a general rule of research, the larger a study is, then the more reliable and trustworthy are the results. If this had been a study of 29 children, you would not be wasting your time right now reading this post. But, studying this many kids for this long a time-period gives this study great weight and the findings must be presented. <br />

What they found was that in children, consuming Junk Food (highly-processed, high-sugar, high-fat) was directly linked to a lowering of IQ (Intelligence Quotient, a standard of measuring a kid's smarts). They researchers of this study balanced the data so that income, location and family situation would be controlled for, thus taking those factors out of the equation. They only studied diet in these children and found out what we all already thought we knew was true, Junk Food is just that, JUNK.<br />

Any teacher will tell you 5 points is a really big deal. 5 IQ points can mean the difference between going to that good college or working at the factory (if there still is one to work at). Kids who ate a healthy, natural diet had higher IQ's in the study.<br />

My generation and the generations since have been brain-washed by TV Commercials to believe that this JUNK food is just a trivial little thing, not harmful, and that you are being a Mean-Parent if you don't let your child have the cereal with the rabbit on the box. We as parents have been seduced by the friendly clown icon of the largest corporate restaurant chain in the world into believing that processed, fake McFood really won't harm our kids; it's all just fun and games, right?<br />

Well, let me speak plainly... Pretending that what you put into your child's mouth is trivial or fun, or not really that important, is deluded thinking. Guess what, everyone likes the taste of sweets and junk food. The multi-national corporations who run the food industry spend BILLIONS each year on all sorts of chemical taste and color enhancers to make their food look and taste good. You are not being a good parent if you let your toddler <i>choose </i>what she will eat, just like you are not a good parent if you let your 10 year-old <i>decide </i>to drive on the interstate, or let you 12 year-old <i>decide </i>to smoke. There are reasons we don't let children Vote, and now you have a proven scientifically valid reason to be the one (the Parent) who choses what your child puts into his/her mouth.<br />

Injections were found to hurt less if you actually watch; in fact, if the body-part being injected is viewed in a magnifying mirror, the injection hurts even less. This may explain why a trick we use for kids who have to get routine allergy shots. We have them blow on the needle as the nurse gives the injection. This helped my oldest son tremendously when he had to get them as a small child. Maybe it wasn't the blowing, just the looking.<br />

In the process of "educating" me, he also gets free research done for himself. I guess that's a fair trade. So off I go to the computer. It was obvious what his opinion on the matter was so I was on a mission to prove him right.<br />

I soon ran into a little problem. After a few hours researching "coffee consumption" on PubMed (the clearing-house of all medical research), I had discovered several key facts:<br />
<br />
<ul><li>Researchers have been trying for decades to prove coffee consumption is bad for you</li>
<li>All Researchers seem to initially believe coffee consumption is somehow bad for you</li>
<li>The studies keep showing, time and again, that coffee consumption is NOT bad for you</li>
<li>Most studies actually show positive effects from coffee consumption</li>
<li>After years of trying, Researchers can't prove that coffee consumption is bad for you</li>
<li>They can't even prove it's bad for that most precious and fragile creature, the pregnant woman</li>
<li>In our lady's case, there was not a shred of evidence to prove what he had told her; actually, the research leaned the other direction</li>
</ul>I relayed this info to my chief resident who seemed unmoved by the evidence. Early next morning, we again visit with our preeclamptic patient, checking her labs and her reflexes;<br />
"Good morning." she said, sipping her coffee.<br />

</ul>I could go on all day. The great irony here is that most of these studies where undertaken to prove just the opposite of the findings. For example, in a <a href="http://www.ncbi.nlm.nih.gov/pubmed/20980333">Finnish study</a>, researchers were pretty convinced that coffee consumption lead to an increased risk of Heart Failure. Poor fellows, after they had crunched all their numbers, they found that not only was their assumption dead wrong, but probably the more coffee one drank, the less likely one was to suffer from heart failure. <br />
<br />
This seems to fit nicely with my theory that if something grows on trees, or in the garden, it is good for you. So, I'm going to pour myself another cup of coffee, and I suggest you do the same.<br />

I will admit that when I was a kid I truly believed that Cheerios would make me stronger, and Frosted Flakes would make me a better basketball player. But, I grew up; I thought we all had. I guess not. Anyway, let's look at just how bad kid's cereals are.<br />

These are ALL sugar and if the company were forced to group these sorts of things together, then most cereals would have sugar as their number one ingredient. Eggs and turkey sausage are easy make a great breakfast that will really have their brains working all day long. Yale actually found that kids will happily eat breakfasts much lower in sugar if they are provided with options.<br />

Soon thereafter, my trusty, usually mild-mannered, Medical Scientist (that's the latest term for lab tech) came to me with a puzzled look on her face and respectfully said, "What the hell is up with all these low Vitamin-D levels, and how did you know to check'em anyway?" She also works in the lab of one of the larger hospitals in the area, and she hadn't seen any other doctors ordering Vitamin-D levels. "It's my job to know", I said wisely as I walked away, not ready to reveal the shallowness of my pool of knowledge on this subject just yet. I tried at lunch that day to think back to medical school...<br />

We had one course in nutrition in medical school. It was a small course, very small. It was taught by a diabetic biochemist from New Zealand who only seemed to want to talk about diabetes and pasta. The paste-thing sticks with me because he said it like, "paaasta". I couldn't remember a thing about Vitamin D from the class and after some digging I turned up the tiny little textbook-let that accompanied the class. There was two pages concerning Vitamin-D (I was right about the pro-hormone thing!) But, that was it. It said nothing about why all the adults in my small Southern town would be Vitamin-D deficient, or what that could lead to (except rickets...) It recommended Vitamin-D-2 as a replacement and that was about it. <br />

We've easily checked levels on over a thousand patients. In the beginning they were all low, and I mean ALL! <br />
<br />
We would consider it a huge deal if all of a sudden 1 out of every 5 patients was abnormal in one certain lab, but 9 out of 10? Out of 100 folks over 65 years of age, 95 would be low, with levels anywhere from 6 to 20. The other five would have a level in the low thirties, just in the normal range. The current normal range of the reference lab I use is from 32-100 ng/mL. <br />
<br />
Since starting to have folks take a daily Vitamin-D-3 supplement (usually 2000 IU/day) I now see many more normal readings on Vitamin-D levels. Sometimes I have a patient who is worried they may be taking too much. It's a rational concern from their perspective, but it is of no concern to me as the prescriber. You see, since starting to prescribe what most would consider a large daily dose of Vitamin-D-3 I have seen exactly ZERO patients have a level above 100 ng/mL. That's right, not a single person has tested above the upper limit cut-off level since I began looking. I have a few patients who take 5000 IU daily and their levels are normal as well. Honestly, the highest Vitamin-D-25 level I have ever seen was in the 60's, no where near the upper cut-off. I'd be interested to hear from any Doc who has seen a Vitamin-D-25 level above 100...<br />

So, you ask, do I really think the Vitamin-D-3 supplements are decreasing the incidence of skin cancers and pre-cancers? Well, I was strongly considering the possibility, trying to think of any other reason why we would be seeing this decrease in skin pre-cancer, when Ms. Truvie made her appointment. <br />
<br />
Ms. Truvie is one of my most beloved patients. She is 93 years old and must have had the fairest skin in all the land, and must have gotten one thousand sun-burns in her life. For you see, Ms. Truvie was covered with actinic keratosis (pre-cancerous skin lesions). She has been my patient for about seven years now and I've lost count of the number of times I've sat and talked with her about the good old days as I froze off one lesion after the other. Her's was one of the first Vitamin-D-25 levels I ever checked, and she clocked in at 6.2; one of the lowest I've ever seen. <br />

I see Ms. Truvie about every 3 months due to her age; to check bloodwork, freeze the worst patch of lesions, and get her refills. For a while I saw the trend of her actinic keratosis being generally smaller, and fewer in number. <br />
<br />
But today I have to admit that her skin is 95% better. There was really nothing that needed freezing, a first for her (which she commented on because she is still very sharp). I stopped Nurse Wendy in the hall and said, "Did you see her face today?" Without even asking who I was talking about she returned a nod and a knowing, "Yep, this Vitamin-D kick of yours is gonna cost you a lot of freeze-money, huh?" <br />

Of course she knows that I'm very happy to find another way to supplement my income besides freezing the flesh of my very nice neighbors. I'm just happy to have been able to jump on the problem early and be making a difference. I've had hundreds of patients comment of improved skin, decreased bone pain, fewer falls, and I have noticed substantially fewer broken hips in our elderly population since starting our Vitamin-D program.<br />

Ms. Truvie's levels now run in the high 50's most times, and she thinks that her legs hurt her less since she has been taking the Vitamin-D as well. Now of course neither Ms. Truvie nor I have studied this scientifically, but common sense is what it is and neither of us can think of another reason for her actinic keratosis to just dry up; not just a few of them, mind you, but almost all of them. So, although Vitamin-D of any kind is not FDA-Approved, I for one will be recommending it to all of my adult patients, and taking it myself every morning.<br />

Two years ago I decided that I wasted more time talking to Drug Rep's (those pretty people in suits you always see in your doctor's office, who often get to cut line in front of you) than it was worth. We started stopping them at the front desk; they could leave samples if they liked, but I would no longer spend any time talking to them. When patients ask for a name-brand there is usually a reason; either they just saw the TV commercial and were so persuaded by the paid actors that they will pay extra for this pill, even though the guy who is trained to make that decision (me) is sitting right there on his stool telling them the generic is just as good, or they had a friend recommend a certain pill and they are eager to give it a try, or they want the name-brand of something for its re-sale value... <br />
<br />
I used to try to reason with such people but quickly realized that TV commercials and non-MD friends evidently seem to give more reliable medical advice that a silly doctor and so I just smile and write them the prescription (except for the re-sale folks). Usually we get a call from the pharmacy in a couple of hours from a flustered pharmacist (and the patient still yelling in the background) who has just made it known that insurance will not cover the televised pill, and that it will cost the patient $256 every month to take said pill. We quickly apologize for <i>our </i>error and change the medicine to the generic, and everyone is again happy.<br />

<b>Patient Time -</b>More than once, out of the corner of my eye I would see some sweet older lady patient eye-balling me as I talked to the pretty Drug Rep who was eating into the patient's time. For a few years I told myself that Drug Reps contributed to my on-going education and this inconvenience couldn't be helped. <br />

The only place you will see pretty Drug Reps in my office today is at the front desk trying to con their way down my hall (and failing). I would much rather help another patient or two in a days time than listen to the canned sales pitch from a Drug Rep.<br />

I thought back to my days of roaming the halls in the Biochemistry and Cell & Molecular Biology Labs and I remembered something; there were very few really cute people there, and none of them were well-dressed! These Drug-Reps are trained and polished to appeal to my weaknesses as a doctor, to study me, take notes about me for their partners to use, and to trick me into making a decision that I might not have made otherwise. In order for one to be a good doctor science has to win over cuteness every time.<br />

<b>Appearance -</b> The little old lady eye-balling me for letting the attractive Drug Rep cut in front of her was right. This drug rep was only going to tell me what his/her company wanted me to know. They would minimize any <i>unfortunate </i>findings in the research data. <br />

Even though I was talking to the Drug Reps with the best of intentions, my patients were basically getting the message that I would rather spend my time talking to attractive, well-dressed Reps than to my patients. That certainly is not true, and I don't want to give any such appearance to my patients.<br />
<br />
<b>What about the Free Samples -</b> Times have changed. In the past those free samples could actually help patients. That was back in the days when Synthroid and Coumadin were brand new. Both these and hundreds of other important drugs are now generic which means no samples are left. Samples nowadays usually are for drugs that are very expensive and are of little more benefit than the generic equivalent. Oh, and they no longer leave any Viagra samples, sorry.<br />
<br />
So much money is spent on Drug Ads that different dosages of the same pill often cost the same amount; this should tell you how much the drug itself really costs. For example, Viagra comes in 25, 50 and 100mg doses which all cost the same amount. How is it possible that the 4x's greater dose of 100mg cost the same as 25mg? It's because the chemical itself is relatively cheap; the multi-millions in Drug Ads, however, are anything but cheap. <br />
<br />
Recently I found the video below, and I found it to be a little disturbing. It was made by a former Drug Rep who has "come clean" about the tactics, strategy and truth-stretching that are common when drug companies communicate with doctors. There is a whole science behind taking a college graduate and training them to walk and talk and act in such a way that doctors feel comfortable around them, and indeed, respect their <i>advice</i>. This lady seems to be pulling out all the stops and I think this information is very important for everyone who trusts their doctor's advicer.<br />

Make no mistake, some patients are quite sick and need to take medicine, some need to take a lot of medicine in order to function at a level meaningful to them. But we all need to remember that TV commercials are not there to educate us, they are there to sell a product. In the case of this product (pills), the company needs a middle man (your doctor) to close the sale. Taking a daily pill is a big commitment and should not be taken lightly.<br />

Email this article to your doctor and ask what he/she thinks...<br />

Feel free to bring a list of questions you have for me, print info off a website, or even bring your friend who recommended the pill; I promise I will listen and together we will decide the best course. It worries me when a doctor is offended by a patient who has been doing some of their own research; every patient should be very knowledgeable about their own condition and the medicine they might need. But, if I'm trying to tell you you don't need a pill, or that a generic pill will do just fine, remember that I make the same amount whether or not you take the name-brand pill; since I don't see Drug Reps, I tend to focus on what is truly best for you and your pocketbook. Trust your doctor, and if you don't trust your doctor, find a new one.<br />

Weight Loss -</b> Although this one costs nothing and can be done with no doctor involved, I didn't put it first or many people would just quit reading at that point. People find this one quite hard - I think - due to the fact they want to alter their diet a little bit and maybe exercise a tiny bit more and then have this really add up to big weight-loss. Sorry, folks, it just doesn't work that way. I do advocate making changes in the diet and your lifestyle slowly, but these changes have to ultimately be substantial and lasting for them to lead to meaningful weight-loss.<br />

<b>4. Walk -</b> This is perhaps the most important, and misunderstood, point on my list. I can't even estimate how many hundreds of times I have had a patient inform me - while I was trying to tell them about the benefits of walking - that their doctor told them not to walk. I haven't tried to chase down every specialist who has supposedly given this advice, but the ones I have asked have told me they would NEVER tell a patient such a thing. Somehow patients mis-hear their doctor tell them not to walk, but I think very, very few doctors ever tell their patients not to walk. <br />
<br />
Now granted, the doctor may have said not to walk for a few weeks after a surgery or procedure, but as long-term advice, I can't imagine any doctor telling any patient not to walk; please let me know if you can find any evidence to the contrary. Walking will NOT wear out the cartilage in the knee any sooner. If anything, walking will stimulate the joint and increase the lubricating fluid in the knee. If you, or your loved one, are completely sedentary, you may need to start with something as simple as walking 100 feet ever day. <br />

After two weeks of doing this every day, you can double the distance to 200 feet or 2-hallway laps. I tell all my truck-driver patients to walk truck-laps; start with one lap around your truck each day, and then increase to 2 laps after two weeks of walking one lap. After two more weeks increase to 3 truck-laps, and so-on adding an extra truck-lap every two weeks. <br />
<br />
By doing this a truck-driver who is completely sedentary can become quite the walker in about six months time, all without over-doing it, over-stressing his heart, or increasing his risk of injury much at all.<br />
<br />
<b>5. NSAIDS -</b> Let me say that I would rather you not take anti-inflammatoriesinflammatories to control their joint pain and stiffness. <br />
<br />
NSAIDS (ibuprofen, naproxen, meloxicam, etodolac, indomethacin, sulindac, etc) have a long list of side effects and should only be used after all other avenues have been tried; but, in my opinion, they are still much less invasive and safer than knee replacement surgery.<br />

Steroid Injections -</b> This treatment is quite a bit more invasive than the other options on this page, and I would highly recommend you try 1-5 above first (and, in that order) before even thinking about a joint injection. During my years of practice I have found knee injections, (with steroids) to be very safe and effective, with very little risk if performed correctly and not used too often. I can find no research that proves steroid injections harm the knee joint if given no more often than every 3 months. <br />

7<b>. PRP Injections -</b> This is a very new technique so I don't whole-heartedly endorse it. An old Family Physician once wisely told me, "Don't ever be the first or last to try something." And within that advice I can say that this new treatment looks very promising, and makes good medical sense, but we will just wait and see. Now there are some older orthopedic surgeons who will give steroid injections monthly or even weekly, but remember they are surgeons and if it damages the joint, then they get to replace it... Many Family Physicians and some Internal Medicine doctors can give these steroid injections just as well, and much cheaper, than orthopedic surgeons can or will. I personally give these injections in my office, but in many orthopedist offices, the assistant or nurse will give the actual injection into the knee...<br />

It is my belief that thousands of people have needless knee replacement surgery every year. The reasons they do this are many; it could be because their doctor misleads them, it may be because they just don't know about items 1-5 above, it might be because they are lazy and don't want to change their life in any meaningful way, or it could be a combination of all these reasons. In any event, I would love to see all these thousands of patients take charge of their knee arthritis and manage it in such a way that they never need knee replacement surgery.<br />
